Sceletium tortuosum has a long history of traditional use by San and Khoikhoi peoples of South Africa for mood, stress, and social purposes. In vitro and animal studies confirm serotonin reuptake inhibition and PDE4 inhibition as primary pharmacological actions. A limited number of small human studies suggest reduced anxiety and improved mood, but large-scale randomized controlled trials are absent. Evidence for individual alkaloid contributions, optimal dosing, and long-term safety remains preliminary.
