ALPHA-PHP
Synthetic cathinone and pyrovalerone derivative acting as a potent dopamine reuptake inhibitor with high abuse liability.
Alpha-PHP is a synthetic cathinone from the pyrovalerone derivative subgroup, structurally related to alpha-PVP (flakka) as its one-carbon homolog. In vitro studies confirm it is at least as potent as alpha-PVP at inhibiting dopamine reuptake, with most analogues showing narrow potency ranges and high DAT selectivity. Forensic case series (29 and 33 cases respectively) document serum concentrations ranging from 1 to 83 ng/mL and 0.75 to 128 µg/L, with nearly universal poly-drug co-consumption including opiates, benzodiazepines, and cannabinoids. Clinical case reports from poison control centers describe severe toxidromes including psychomotor agitation, hallucinations, tachycardia, hyperthermia, and rhabdomyolysis. Animal studies reveal sex-related differences in pharmacokinetics and behavioral effects, with female mice showing greater sensitivity to sensory and thermoregulatory effects. Evidence remains limited to forensic case reports and preclinical studies, with no controlled human pharmacological trials.
