Baclofen is FDA-approved for spasticity and is the only approved GABA-B agonist. Intrathecal baclofen is well-established for severe spasticity of spinal origin. For alcohol use disorder (AUD), evidence is mixed: a Cochrane review found no significant difference vs placebo for primary outcomes, while observational studies using tailored doses report good effectiveness. A meta-analysis of 14 RCTs found only a small, non-significant superiority over placebo. High doses (up to 300mg/day) are used in France for AUD but carry increased risk of severe adverse effects including sedation, respiratory depression, and psychosis.
